Important Notice
In order to be financially prepared at the time of your visit, please contact your insurance company before your arrival to inquire about your financial responsibility concerning copays and deductibles pertaining to your visit.
How to Make an Appointment
Scheduling an appointment in advance assures that you will have minimal waiting when you arrive and that the appropriate preparation are already made for your visit.
Should you have an emergency, we will do everything we can to accommodate you. Please let us know as early as possible that you need to be seen on the same day. When making your appointment, let the staff know the nature of you complaint so that adequate time will be devoted to your care.
If for some reason you can not keep your appointment or will be late, we ask that you give us as much notice as possible. The office will charge a $25 fee for appointments that were are not kept or not cancelled.

Phone Calls
Our office hours are 9:00 am to 5:00 pm, Monday through Thursday, and 9:00 am to 3:00 pm on Friday. We are closed on weekends. You may reach our office by calling 301-540-4400. Should you need to reach the doctor after hours, call the office number and ask the operator to page the doctor for you. He will return your call as soon as possible.
If you have a life-threatening situation, call 911 and proceed to the closest emergency room. If you are pregnant, go to Labor and Delivery at Shady Grove Adventist Hospital in Rockville. Please remember that Dr. Kazemi will only attend to that hospital.
Prescription Refills and Laboratory Results
Regular prescription refills will be handled during regular office hours. Please have the preferred pharmacy number available so that we can request a refill as soon as possible.
Please make sure you have a sufficient amount of medication available before calling for a refill. Some medications require a follow-up visit before refill. To inquire about current lab results, call the office during regular office hours.
Fees and Payments
We are more than happy to discuss our fees with you and give you a cost estimate in advance. When making appointments, please give staff your insurance information. A staff member will contact your insurance company and obtain the necessary financial information for you. We participate with most insurance plans and are happy to assist you in accessing your insurance company’s fee structure. Most insurance carriers pay only a percentage of the usual fee and patients are responsible for the balance due (copay). If you are not sure of your balance, you should call your insurance company in advance. Your balance is due at the time of service and can not be billed at a later date.
Please inquire whether a primary care referral is required for your visit. We are unable to see patients without a necessary referral.
Insurances
We accept almost all the private insurances as well as Maryland Medicaid and all the subsidiary insurances of Medicaid, except United Healthcare. We also do not participate with Medicare Insurance.
